Most importantly for exhausted family caregivers, GUIDE covers professional in-home respite care services at absolutely no cost.

Eligibility Requirements

You Loved One May Be Eligible If:

They have dementia (diagnosis can be provided)

They have Traditional Medicare Parts A & B

They live at home or in an assisted living facility

Not Currently Eligible:

Those enrolled in Medicare Advantage

Those in hospice care (palliative care is OK)

Those in long-term skilled nursing (independent living, assisted living, and memory care facilities are OK)

What is the CMS GUIDE Model?

The GUIDE (Guiding an Improved Dementia Experience) Model is a Medicare initiative that provides comprehensive, coordinated care for people with dementia and their caregivers. The program provides free comprehensive assessment, caregiver training, resource navigation, and in-home care. GUIDE covers all types of dementia (e.g. Alzheimer’s disease, vascular dementia, Lewy body dementia, frontotemporal dementia, etc.). If you do not have an existing dementia diagnosis, our partner clinicians can help you get a diagnosis to be eligible for GUIDE.
